New PCMCH Resources to Support Early Pregnancy and Postpartum Care

The Provincial Council for Maternal and Child Health (PCMCH) is pleased to announce the release of two new resources for patients and healthcare providers to support continuity of care during early pregnancy and the postpartum period: the Early Pregnancy Concerns Care Navigation Resource and the Postpartum Warning Signs Information Sheets.

Together, these resources support clearer communication, consistent follow-up and a more patient-centred experience from early pregnancy through the postpartum period.

Early Pregnancy Concerns Care Navigation Resource

This patient-facing resource is intended for individuals experiencing early pregnancy symptoms such as pain, bleeding or uncertainty about how pregnancy is progressing. It provides clear, accessible information to help individuals navigate the healthcare system, understand what follow-up may be necessary, and become familiar with common medical terms used during early pregnancy assessment. It also clarifies when urgent or emergent care is needed and provides additional information to direct individuals to emotional, cultural and community supports.

The Early Pregnancy Concerns Care Navigation Resource was developed to address challenges in care navigation identified at a provincial workshop, hosted by PCMCH in 2024 in collaboration with the Pregnancy and Infant Loss (PAIL) Network and the Better Outcomes Registry & Network (BORN) Ontario. Postpartum Warning Signs Information Sheets

Developed for patients and healthcare providers, the Postpartum Warning Signs Information Sheets support care beyond pregnancy.

The patient-facing information sheet is designed for anyone who has been pregnant, experienced a pregnancy loss, or given birth in the past 12 months. It uses plain language to help individuals and families recognize concerning symptoms and know when to seek emergency or follow-up care. This information sheet is available in the following languages:

The provider-facing information sheet supports healthcare providers in identifying postpartum complications and reinforces the importance of including recent pregnancy in differential diagnoses. It’s available in English and French.
